Abstract
Restriction of information, superfluous items, unsuitability of form, and expense are some arguments against the use of printed forms. Advantages of such forms are good organization of case record and more uniform and easily available data for many purposes such as research and follow-up. The form suggested here is a twelve-page pamphlet including the following major headings: identification, summary, developmental history, parents and family, home, child's behavior, test performance, physical examination, delinquency, and speech defects.
